We present the case of an 11 year-old-boy with frequent, predominantly nocturnal episodes, initially interpreted as generalized seizures, with no response to sodium valproate. Video-EEG during sleep showed a seizure characterized by asymmetric tonic posturing of upper limbs followed by left lower limb cycling movements, with right hemispheric epileptiform discharges at onset of the episodes. The patient had a concussion three years earlier, and brain MRI was normal. Video-EEG findings supported replacement of anticonvulsant drug to oxcarbazepine, which resulted in complete remission of seizures.
