Ética nas decisões sobre o fim da vida - a importância dos cuidados paliativos

| dc.description.abstract | Quando a Medicina Curativa já não produz qualquer alteração no quadro clínico de um paciente, cujo estado de irreversibilidade é definitivo, torna-se urgente dar assistência àqueles que se encontram em fase final de vida. E porque, não raras as vezes, são acompanhados de uma enorme dor e sofrimento que contribuem para a perda de qualidade de vida, a Medicina Paliativa procura travá-los. Os Cuidados Paliativos existem enquanto conjunto interdisciplinar direcionado para o acompanhamento continuado do doente terminal. São uma resposta ética, médica e psicológica face às necessidades últimas do estado do paciente. Mas não podem nunca ultrapassar o limite da dignidade do ser humano e as suas escolhas. Isto é, é admissível sim que se mantenha a qualidade de vida mas não quando os esforços são desproporcionais e inúteis e contrariam em absoluto o previsto, por exemplo no Testamento Vital do paciente. Em Portugal a Medicina Paliativa tem vindo progressivamente a ganhar espaço, físico (crescente número de unidades de cuidados paliativos espalhadas pelo país) mas também ao nível da consciencialização dos governantes (para assegurar a proliferação e assistência logística aos cuidados paliativos). Pretende-se que o doente terminal, independentemente das suas condições económicas e sociais, possa aceder à Medicina Paliativa e ser sujeito pleno do direito à saúde. Os princípios bioéticos da Autonomia, Beneficência, Não-Maleficência e Justiça compadecem-se com os cuidados paliativos a que cada doente aceita submeter-se, a que tem o direito de aceder para maximizar os benefícios do tratamento e minimizar os danos. | por |
| dc.description.abstract | ABSTRACT When Curative Medicine no longer has any effect on a patient’s clinical picture because his state is definitely irreversible, it becomes urgent to give assistance to anyone who is in the final stage of living. And because, quite often, this phase is preceded by enormous pain and suffering, which contributes to the loss of quality of life, the aim of Palliative Medicine is to fight it. Palliative Care exists as an interdisciplinary set directed at accompanying the terminal patient. It is an ethic, medical and psychological response to the final needs of the terminal patient. But we should never overstep the boundaries of human dignity and the patient’s choices. That is to say, it is acceptable to maintain the quality of life, but not when the means are disproportional, useless or absolutely contradict what is contemplated, for example, in the patient’s Living Will. In Portugal, Palliative Medicine has progressively gained space, both physically (through an increasing number of palliative care units through the country) and in terms of government awareness (to assure the proliferation and logistic assistance to palliative care). The aim is that the terminal patient, regardless of his economic and social conditions, has access to Palliative Medicine and be fully entitled to his right to health. The bioethics principles of Autonomy, Beneficence, Nonmaleficence and Justice sympathize with the palliative care which each patient agrees to submit to and to which he is entitled in order to maximize the benefits of the treatment and minimize its damages. | por |
