RESUMO
As autoras descrevem o caso clínico
de uma adolescente de 17 anos, internada
por episódio maníaco com início
durante um período de tratamento com
claritromicina, e alguns dias após a administração
intramuscular de betametasona.
O pai apresentava queixas compatíveis
com patologia do espectro bipolar,
e os pais eram consanguíneos.
É discutida a possibilidade de se tratar
de uma Perturbação do Humor Induzida
por Substância (DSM-IV TR). A esse
propósito, faz-se uma revisão sumária
da literatura referente a casos de mania
induzidos por antibióticos, um síndrome
que alguns autores denominaram de “Antibiomania”.
Salienta-se, além disso, a
predisposição genética e a corticoterapia
recente como possíveis factores de risco.
Espera-se que este caso clínico
constitua um alerta para a possibilidade
de desenvolvimento de um episódio maníaco
durante o tratamento com antibióticos,
particularmente com a claritromicina.
Será necessário que a investigação futura
esclareça múltiplos aspectos relacionados
com a incidência, os factores de
risco e o prognóstico desta situação.
ABSTRACT The authors describe a case of maniac episode in a 17 years old female, initiated during a period of treatment with clarithromycin and some days after the intramuscular administration of bethametasone. Her father had symptoms compatible with bipolar spectrum disorder, and her parents were cousins. The possibility of being a Substance-Induced Humor Disorder (DSM-IV TR) is discussed. The literature concerning cases of antibiotic-induced maniac episodes, sometimes called “Antibiomania”, is briefl y reviewed. The genetic predisposition and the recent use of corticotherapy are underscored as possible risk factors. It is expected that this case study constitutes an alert to the clinicians towards the possibility of developing an antibiotic-induced maniac episode, particularly during treatment with clarithromycin. It will be necessary that future investigation elucidate multiple aspects regarding this situation.
