- Acral necrosis by Stenotrophomonas maltophiliaPublication . PEREIRA, O.; VELHO, G.C.; LOPES, V.; MOTA, F.; SANTOS, C.; MASSA, A.Keywords:necrosis;skin and soft tissue infection;Stenotrophomonas maltophilia Abstract Background Stenotrophomonas maltophilia (SM) has been considered a nosocomial pathogen. Nevertheless, community acquired infection may occur more frequently than usually recognized. Case We describe distal necrosis of the fingers by SM in a farmer, contracted in the community and successfully treated with a combination of cotrimoxazole and ciprofloxacin. The patient was diagnosed with chronic lymphocytic leukaemia 6 months later. Conclusions This unusual presentation shows that infection with SM should be included in the differential diagnosis of the skin and soft tissue infection, even in apparently healthy patients.
- Larva migrans cutânea em idade pediátrica: a propósito de um caso clínicoPublication . Ferreira, C.; Machado, S.; Selores, M.A Larva Migrans Cutânea (LMC) é uma dermatose infecciosa provocada por parasitas nemátodes que, habitualmente, não infestam o Homem. É uma doença endémica em países tropicais, de diagnóstico pouco comum noutros países. Os autores descrevem um caso clínico de LMC numa criança do sexo feminino, de 3 anos de idade, regressada de uma viagem a zona endémica, com lesões serpiginadas típicas em ambos os pés. A evolução foi favorável com a terapêutica instituída (Albendazol oral e Tiabendazol tópico). A propósito deste caso, os autores fazem uma revisão da epidemiologia, manifestações clínicas e opções terapêuticas em idade pediátrica. Cutaneous Larva Migrans (CLM) is an infectious dermatosis caused by nematodes parasites, which do not usually parasite men. It’s endemic in some tropical and subtropical areas and rare in industrialized countries. The authors present a case of CLM in a female child aged three, which had recently travelled to an endemic area, with typical serpiginous lesions affecting the feet. The child was treated with oral Albendazol and topical Tiabendazol with complete resolution of the complaints. The authors review the epidemiological and clinical features and summarise the therapeutic options in pediatric ages.
- Terapêutica Tópica em Dermatologia PediátricaPublication . Rocha, N.; Horta, M.; Selores, M.RESUMO Todos sabemos que as crianças não são meramente pequenos adultos. Em comparação com aqueles, estas apresentam uma relação superfície cutânea/ peso corporal aumentada que contribui, por um lado, para aumentar a susceptibilidade à irritação, por outro, para maximizar a absorção percutânea com consequente aumento do risco de toxicidade sistémica. Tendo em atenção os aspectos particulares da pele da criança, os autores fazem uma revisão das principais terapêuticas tópicas usadas em Dermatologia Pediátrica, nomeadamente corticosteróides, anti-infecciosos, imunossupressores, emolientes, esfoliantes, cremes barreira, adstringentes, antissépticos, anti-histaminicos, antiacneicos, antipsoriáticos, anestésicos e protectores solares. ABSTRACT We all know that children are not just small adults. Children have a bigger cutaneous surface / corporal weight relation. This contributs, on one hand, to increase the irritation susceptibility, and on the other to maximize transcutaneous absortion, leading to a greater risk of systemic toxicity. Considering the particular characteristics of children’s skin, the authors review the main topical treatments used in Pediatric Dermatology, namely corticosteroids, antimicrobials, imunossupressors, emollients, exfoliants, protective creams, adstringents, antiseptics, antipruritics, antiacneics, antipsoriatics, anesthetics, sun protectors.

Rocha, N.; Selores, M.RESUMO A dermatite das fraldas deve ser interpretada não como uma entidade diagnóstica específica, mas sim, como um diagnóstico de localização, que engloba um amplo grupo de dermatoses de etiologia multifactorial. Neste trabalho, será abordada principalmente a dermatite da fralda irritativa primária ( para alguns autores sinónimo de dermatite da fralda), os seus principais diagnósticos diferenciais e tratamento. ABSTRACT Diaper dermatitis is a topographic description of a wide range of inflammatory processes that occur in the area covered by a diaper. The authors describe mainly the primary irritant diaper dermatitis (also called diaper dermatitis) its differential diagnosis and treatment.

Ferreira, M.; Machado, S.; Selores, M.RESUMO A exposição solar é um factor de risco conhecido para o desenvolvimento de tumores cutâneos, incluindo melanoma maligno. O ser humano, e em particular a criança, gosta do sol e no seu dia a dia é exposto a radiação solar enquanto trabalha, brinca ou joga. A infância e a adolescência são períodos críticos para o subsequente desenvolvimento de tumores cutâneos, devendo a protecção solar ser iniciada o mais precocemente possível. Neste trabalho, realizado com base na revisão bibliográfica, pretende-se rever a actuação da radiação solar e as formas de protecção contra os seus efeitos nocivos.ABSTRACT Sun exposure is a known risk factor for the development of cutaneous tumours, including melanoma. The human being, and specially the children, enjoy the sun, and usually suffer an exposition to sun radiation while working or playing. Childhood and adolescence are critical periods in the aetiology of subsequent cutaneous tumours. Sun protection should be beginning since infancy. This article, based on a literature review, has the purpose of describing the sun effects and the appropriate preventive measures.
- Eruptive syringomasPublication . Teixeira, M.; Ferreira, M.; Machado, S.; Alves, R.; Selores, M.Eruptive syringoma is a rare clinical presentation of a benign tumor of the eccrine ducts. It consists in successive crops of small skin-colored papules on the anterior body surfaces. It generally occurs in the peri-pubertal period. Treatment of this benign condition is cosmetic only. A case of a 19-year-old female with a 5-year history of eruptive syringoma is presented.
- Leukemia cutis resembling a flare-up of psoriasisPublication . Ferreira, M.; Caetano, M.; Amorim, I.; Selores, M.Abstract Leukemia cutis represents a skin infiltration by leukemic cells. Clinically it can mimic a wide variety of dermatoses. We describe the case of a 64-year-old man with psoriasis who presented with a 4-day history of erythematous, slightly scaly, asymptomatic plaques distributed on the trunk and upper-extremities, and associated asthenia, myalgias, and anorexia. A skin biopsy revealed a leukemic infiltrate. Studies of peripheral blood and bone marrow provided a diagnosis of acute monocytic leukemia. This case report shows the importance of the clinical suspicion for the diagnosis of leukemia.

- Allergic contact dermatitis to Aloe veraPublication . Ferreira, M.; Teixeira, M.; Silva, E.; Selores, M.Abstract We present the case of a 72-year-old woman observed for dermatitis on the legs followed by apperance of erythema on the eyelids. She had a past history of peripheral venous insufficiency and had been using self home-made Aloe vera juice over the legs for relief from pain. Patch tests showed positive reactions to the leaf of Aloe, the macerated Aloe jelly, and nickel sulfate. Although most manufacturers process Aloe products avoiding its irritant extracts, and probably as a consequence reports of allergic reactions are rare, one must remember that the growing popularity on the use of Aloe products may stimulate its use ‘as is’ by the patients. Furthermore, it is important to specifically ask patients about the use of these products, because they consider it as innocuous and thus would not spontaneously provide such information
- Calcinosis cutis: A rare feature of adult dermatomyositisPublication . We describe a case of a 55- year-old woman with dermatomyositis who presented with dystrophic calcinosis resistant to medical treatment.
